For Senior adults 60+ who live in Portage County and are referred by Adult Protective Services

Services Provided:

- Home visits
- Information and Referrals to agencies
- Case Management
- Coordination of Services
- Assist with applications for various types of assistance
- Assist with finding new housing
- Placement services
- Assist with Medicare and Medicaid Applications
- Transportation
