Portage Area Transitional Housing
The PATH program assists homeless families and individuals who are experiencing significant difficulties that hinder their ability to maintain stable housing. PATH offers comprehensive, supportive services and subsidized transitional housing for up to 24 months to support participants in their efforts to increase social and economic strength and to secure permanent, stable housing.
Mission
To provide an integrated, comprehensive variety of services and case management, in conjunction with housing, for homeless families and single adults. The services are designed to promote greater self-sufficiency, self-determination and permanent housing acquisition.
Goals
The goals of the PATH program are as follows:
- To increase family/individual economic and social empowerment
- To build on family/individual strengths by improving functioning
- and self-determination
- To assist participants in achieving and maintaining permanent housing.
Eligibility
Who is eligible? - Homeless families and single adults who desire to become self-sufficient and are willing to work with an Advocate/Case Manager, and Housing Counselor to devise and carry out a plan for self sufficiency and permanent housing.
Housing
PATH housing for families with children is available at dedicated sites in Ravenna, Kent and Windham. PATH housing for single adults with no children is available throughout Portage County via private landlords willing to accept the PATH rental subsidy.
